Fixed a bug in the reading table of contents code.
There was a bug in the code that reads the table of contents off
the CD. All frame offsets were being read as zero - this could
result in the start of one track overlapping the end of the
previous track by up to 1 second when reading tracks in batch
mode.

Release 1 should support both ATAPI and SCSI drives. However I don't have a SCSI drive to test with, so it may or may not work.
